I/We certify that the above information is correct. I/we give permission for my/our child to participate in the Cheer Passion Allstar program. I/We are aware that Cheer Passion is a non-profit organization. We understand that the medical insurance covering my/our child is required and is my responsibility. I/ We, the parents/guardians of the above named participant of Cheer Passion, hereby give our approval to participate in any and all activities and understand that participation in this cheerleading program may release, absolve, and agree to hold harmless the Cheer Passion Allstars program, the organizers, sponsors, coaches or other participants for any claim arising out of any injury to our/my child. We understand that Cheer Passion’s directors, officers and coaches have no obligation to pay any medical, dental or hospital bills which may be incurred by the cheerleader. We also waive any and all rights, claims, actions, suites and causes of actions that may have to be made on our behalf as a result of any injury at any event/function. A completed emergency form will be required for all athletes once the practice season begins.
$75.00 Commitment Fee with a multi cheerleader discount (1st $75.00, 2nd $65.00, 3rd $55.00). No refunds will be issued once registration fees have been received. I/We understand that 50% of practice wear is due on June 27th and those fees are also not refundable after the fees have been collected.
